For many people, staying in familiar surroundings at the end of their life is incredibly important. Our community palliative care services are here to help make that possible for you and your family.

As a specialist palliative care service, we work closely alongside your GP, hospital team, and other NHS services to provide expert advice. We do not replace these services; instead, we offer them extra guidance and support to help them care for you. Depending on your needs, we may also ask other healthcare professionals to visit you and provide specific care following our clinical advice.

Hospice at Home

Choosing to spend your final days at home is a deeply personal and meaningful decision. Our Hospice at Home service is designed to wrap around you and your family during this time, providing a dedicated layer of hands-on nursing care, intensive symptom management, and emotional reassurance right in the comfort of your own surroundings.

This service is specifically available for individuals who meet the following criteria:

  • You are already under the care of Hospice Isle of Man.

  • You have expressed a clear wish to remain in your own home.

  • You have an expected prognosis of one to two weeks of life.

By managing complex clinical needs and providing sensitive personal care, our team aims to lift the heavy practical weight of the caring role from your loved ones' shoulders. This allows your family to step back from being full-time carers and focus on what matters most - spending precious, quality time together. We work dynamically alongside your wider healthcare team to ensure your dignity is protected, your symptoms are carefully controlled and your individual choices are fully honoured.

Our Clinical Nurse Specialist Team

If you or your loved one has more complex palliative care needs - whether for a short period or over a longer duration - you may require specialist assessment, advice, and support.

This dedicated care is provided by our Clinical Nurse Specialists (CNS). As senior healthcare professionals who specialise in palliative care, they focus on helping you manage your condition and maintain your quality of life at home. Our CNS team can support you with:

  • Complex symptom management: Helping to manage symptoms such as pain, nausea, vomiting, or constipation. This may include recommending changes to your medication, suggesting other treatment options, or discussing referrals to specialist support.

  • Listening and emotional support: Providing a safe space for you and those who care for you to discuss problems, feelings, and the future. This includes helping you create an Advance Care Plan, ensuring everyone understands what to expect and that your choices are respected.

  • Referrals to wider services: Connecting you to additional support when appropriate, either within Hospice Isle of Man or through external organisations that can help.

In January 2021, my Dad, Nigel Cubbon was looked after by the Hospice at Home team. They were absolutely fantastic. At the most devastating and heart-breaking time in my own and my family’s lives, the nurses were compassionate, calm and reassuring.

The nurses also took the time to make sure myself and my Mum, Liz were as ok as we could be. We felt we were genuinely cared about as a family and that my Dad was important to them. It was also reassuring to know the team were only at the other end of the phone if there were any issues or concerns.

In his final days Dad was made comfortable and peaceful by the amazing Hospice Nurses, at home, surrounded by his loved ones.
